Chapter Information Forms
Chapter Presidents, primarily, are responsible for ensuring Greek Life has accurate information on file for the chapter.
The following forms should be submitted any time between May 1 - July 1 during Registered Student Organization (RSO) renewal process. However, if changes occur to any of these forms during the academic year, the Chapter President should re-submit a new form.
