Immunization exemption policy

Students may request a medical exemption or an exemption based on sincerely held religious beliefs.  

Medical Exemptions

Medical exemptions are for students unable to receive vaccines due to health risks. Students must complete the Medical Exemption Request Form and upload it on the Upload tab in HealtheConnect.  

Religious Exemptions

Religious exemptions are for students who choose not to receive or complete a vaccine series due to sincerely held religious beliefs.  Students must complete the Religious Exemption Request Form and upload it on the Upload tab in HealtheConnect.
